Education Consent. Act 99, 2003 Legislative session

Grandparents need informal consent to enroll their grandchildren in school. The caregiver consent for educational law enables grandparents and adult caregivers over the age of 18 to enroll children in public schools. This consent also allow for caregivers to authorize activities such as field trips and after school activites.


Medical Consent
Act 208, 2005 Leguslative session

This act  enables grandparents and adult caregivers over the age of 18 to consent for medical services. Services such as immunizations, dental, vision and mental health care. An authorized affidavit must be completed.


Consent Affidavits

The caregiver consent affidavits do not interfere with parental rights. Rather, it allows current caregivers to provide education and healthcare for our keiki.
